c) poor training form, make sure you select the right weight.. leave your ego at home and go with lighter weights and perform the exercise with proper form. slow and controlled. time under tension!
d) over training, includes: too frequent trainings, no rest days, too much cardio, too many forced reps or training to failure, trainings too long (more than an hour),...
